CULTIVATING PRESENCE: TRAINING RETREAT IN COMPASSIONATE END-OF LIFE CARE

Registration Information

We anticipate this training will fill quickly. We encourage you to apply early. Anyone wishing to attend this training must complete a simple application and make a deposit of $200. All information will be kept confidential.

Application Approval

1. Faculty members will review both online and printed applications.
2. We will start reviewing applications immediately. You will receive a letter or email regarding application approval. Approval letters will include a request for full payment.
3. If approved you must submit full payment before June 15, 2010 to reserve your place.
4. Applications for commuter places will be processed after all 55 residential places have been filled. Please indicate your interest in a commuter place by checking the appropriate box on the application.
5. Final acceptance and logistic information will be emailed or sent by US mail on receipt of full tuition payments.

You may complete your application online. Please complete all questions on the application. You will need a Visa or Mastercard credit card to make your deposit or pay in full. A credit card processing fee is added to all credit card payments (e.g Deposit of $200 plus 3% processing fee=$206).
